After dealer criteria has been met, there must be an initial order of $300.00 (three hundred dollars) placed to be set up as a "Dealer". To continue "Dealer" status there must be a minimum of $500.00 (five hundred dollars) per year purchased. A yearly review will be done, at which time, if yearly minimums are not met it will be grounds for the account to be made inactive. An account that is made inactive will be required to reapply for "Dealer" status and will be required to place the initial minimum order. Any missing information on this application will automatically be cause for denial.

NOTE: This dealer application will be sent to Milwaukee Iron Inc. upon submission. We require a digital signature, or a fax with an original signature in order for your account to be set up. If you fax your application in, please attach pictures of your shop and send through the mail.
